South Manchester services

Welcome to South Manchester's Integrated Neighbourhood Teams.

We are co-locating community health & social care services into four Integrated Neighbourhood Teams (INT’s). We've also got our Urgent Care Service, care navigator roles and Single Point of Access referral centre (SPA), which operates parallel to our four INTs.

We've developed processes and procedures for each INT and we're looking at how to involve administrative functions, mental health and voluntary community sector.
