Specialist Palliative Care Service (Macmillan) – South Manchester
This service supports patients with active, progressive diseases, and a limited prognosis.
The team is dedicated to providing support to meet patients’ complex physical, psychosocial and/or spiritual needs, to enable them to have the best quality of life possible.
Patients who are in the last few days of life and in the last stages of illness with specialist palliative care needs. This includes malignant diseases, non-malignant diseases, multiple illnesses, dementia, and frail elderly people, defined as over 65 years of age with two long-term conditions or more.
We also provide support for family members and informal carers of patients who are dying. Patients with a life-limiting illness who do not have a limited prognosis may also be considered for specialist palliative care if their carers need support.
