Community Adult Learning Disability Service (Health) – Citywide

As an integrated, multi-disciplinary team, we bring together expertise from Occupational Therapy, Speech and Language Therapy, Learning Disability Nursing, Physiotherapy, Psychology, Psychiatry, and Administration. Working in partnership with Adult Social Care to provide high-quality healthcare tailored to individual needs. Our service provides holistic, person-centred assessments and interventions designed to promote improved health, wellbeing, and inclusion.

We deliver high-quality specialist health services that promotes inclusion, wellbeing, and health equality for adults with learning disabilities. We achieve this by empowering individuals and reducing health inequalities across our community.

  • Adults aged 18 and over who have a Learning Disability and associated health needs.
  • Be registered with a Manchester GP

For someone to have a learning disability, all three factors must be present:

  1. Significant difficulties understanding information or to learn new skills (i.e. Significantly impaired intellectual functioning – IQ below 70)
  2. Significant difficulties coping independently (impaired adaptive functioning in at least 2 major domains e.g. independent living skills, self-care skills, etc)
  3. Difficulties must have started in early childhood/before the age of 11 years old and had a pervasive effect on development.

There are 3 community teams across North, Central and South Manchester.

1 Short Breaks (respite service) based in South Manchester.

The service offers a range of specialist health assessments and interventions to meet the complex needs of people with a Learning Disability and their support network.

  • Provide specialist assessments and interventions for a wide range of health needs.
  • Offer support for both physical and mental health issues, including anxiety, emotional difficulties, and complex behavioural needs.
  • Support with daily living skills, such as improving communication, self-care, and independent living.
  • Collaborate with families, carers, and other professionals like GPs and social workers to achieve positive outcomes.
  • Work to reduce health inequalities and support individuals in accessing mainstream services.
  • Offer training and advice to family carers and other support staff.
  • Support for people with long term health conditions e.g. dysphagia, dementia, and respiratory issues.

Anyone can refer to the Community Learning Disability Teams. This includes the person, the GP, social worker, a health professional, advocate, carer or family member.

Referrals are processed through a single point of access for all three teams. A daily referral hub, attended by representatives from our city-wide multi-disciplinary team, ensures timely review and coordination of new referrals.

We are available Monday to Friday between the times of 8:30am – 4:30pm.

Our service is not open at the weekends and bank holidays.
