Community Adult Learning Disability Service (Social Care)

We are a person-centered assessment and support planning team. We will work with citizens to discuss their strengths and areas of support. We will work with them and those who know them best to empower them to have the support around them that they need.

We support people over the age of 18 where their presenting need is related to a learning disability. A learning difficulty or autism does not mean a person has a learning disability·

Adult social care helps people where they might struggle in everyday life because they have a disability or if they are an older person.

A learning disability is something that affects someone for their whole life, it might mean they take longer to learn something. For some people it can mean thy find some everyday tasks difficult – for example cooking, cleaning, keeping themselves safe.

We are social care assessors and social workers.

We will talk to you about your social care needs and wellbeing.

You can tell us what you are worried about.

We will work together to help you.

We can help you decide what you want to learn more about.

We all need help with some things.

We can help you decide what you might want to get better at.

We can give you easy read information about your health or about money or the rules when you rent a house.

We can work together to find out how you look after yourself. This finding out is called an assessment.

We will assess how you look after yourself. Things like washing, bathing, planning your meals, shopping, making meals and looking after your money.

We will assess how you look after your home. Things like hoovering, dusting and cleaning the toilet.

We can help you stay safe at home. We can do this by helping you make your home safer, tidier or cleaner.

We can talk to you and your family or carers about your life and what care and support you might need.

The plan will help you to think about being as independent as possible; learn new things; be safe; have important relationships with people in your life.

Together we can make a plan called a Support Plan.

We are 5 teams based across 3 sites in the city.

Referrals are made via the Contact Centre (0161 234 5001 or email at mcsreply@manchester.gov.uk) or through the front door.

People can be referred by a third party (consent is needed or a best interests).
People can self-refer.
