MEAP Assessment Team

The Assessments Team sits within Manchester’s Equipment and Adaptations Partnership (MEAP)

We assess a person in their home environment, asking how they manage on a day-to-day basis and observing mobility, transfers, and functional tasks.

We provide services to Manchester people of all ages. We work with people with a variety of physical and mental health conditions that impact on their day-to-day living.

We look at how a person manages:

  • getting on/off their chair, bed, toilet;
  • getting in/out of their bath or shower
  • getting around their property and up/down the stairs
  • access in/out of the property
  • washing/dressing & kitchen tasks etc.

We follow an incremental approach, so at first, we look at whether the person needs equipment and minor adaptations. If this will not meet the person’s assessed needs, we may recommend major adaptations such as ramps, ceiling track hoists, wet room shower facility etc.

We also:

  • assess for complex equipment including specialist seating and bathing equipment.
  • provide moving and handling assessments, including provision of moving and handling equipment (eg mobile & gantry hoists, slings, stand aids), and providing training to family members and carers
  • undertake Blue Badge assessments, via desktop or clinic appointments, for applicants who do not meet the automatic eligibility criteria.

An assessment of the person will take place and adaptations are assessed on a sliding scale starting with equipment.
