Manchester Case Management (MCM) – North Manchester

Manchester Case Management (MCM) North specialises in the management of Long-Term Conditions. The team consists of physical health nurses, Mental Health Nurses, Substance Misuse Practitioners, Social Workers and Support Workers. MCM are part of an Integrated neighbourhood team across 4 neighbourhoods in North Manchester, with District Nurses, Care Navigators, Wellbeing Advisors and Pharmacists.

The aim of the service is to improve the lives of people in North Manchester living with complex Long-Term Conditions (LTCs), improving access to required services and reducing hospital admissions. MCM also work with wider community teams to achieve this.

This is a non-emergency service which specialises in helping people with the chronic management of long-term conditions. Patients are 18 years or over and registered with a North Manchester GP. MCM work according to the patient’s health goals providing education, promoting self-management and signposting to required services.

MCM carry out full holistic assessments and are trained in asset-based conversations, drawing on self-management abilities and the health goals of the patient.

Opening hours

Monday to Friday, 8am to 5pm. (No Bank Holidays or Weekends)

Patients do not self-refer. Referrals to the service can be made by community teams such as Crisis Response as well as District Nurses and GPs. Please find a referral form below.
