Children’s Palliative Care Team (Star Team)

Palliative care is patient centered rather than disease focused, death accepting but also life enhancing, a partnership between the patient and family, concerned with managing rather than curing.

The Children’s Palliative Care Team, also known as The Star Team, comprises of nurses, short break facilitators, child & family/ family liaison practitioners and a community paediatrician with a specialist interest in Children’s palliative care.

Hours:

Monday to Friday 9am – 5pm (the team provides a 24hr on call service for end of life patients).Short breaks are provided across seven days according to need.

How to refer:

Referrals are accepted by email, telephone (Monday – Friday), face to face and post. Please telephone the team when making urgent referrals.

Referrals are accepted from:

  • GPs
  • Colleagues, fellow professionals, voluntary, statutory and non-statutory organisations
  • Individuals and families (self-referral)
  • Hospital Paediatricians
  • Community Paediatricians
