Children’s Complex Care and Ventilation Team

The Children’s and Young People’s Complex Care Team is part of the Manchester Children’s Specialist Services Community Nursing Team. The nursing team complete a health needs assessment for children with complex needs who require continuing care. Following assessment, the team will contact the relevant commissioning body for allocation of the package of support.

We provide care for children and young people who have underlying medical conditions and require care and nursing support within their own home. The service is adaptable, and care is provided for children and young people with a range of conditions including ventilation and those with acquired brain injuries.

The team consists of nurses, nurse associate’s and senior nursing assistant who work together to provide the care within the homes. We work collaboratively alongside all of Manchester’s health and social community services as well as working very closely with the Royal Manchester Children’s hospital especially the long-term ventilation unit.

We help children with complex health needs, who are registered with a Greater Manchester GP between the ages of 0 – 16, with support to 18 if in transition.

  • Continuing health care assessments
  • Nursing review and clinical oversight of children under caseload
  • Nursing review of children on invasive and non-invasive ventilation
  • Clinical nursing oversight of children on a personal health budget (PHB)

Referrals can be made via email from a wide range of health professionals from different services or the child’s individual social worker.
