Intermediate Care Unit – Crumpsall Vale – North Manchester

Crumpsall Vale is an Intermediate Care Unit that is designed to provide a rehabilitation service with input from a team of dedicated professionals.

It is a service which promotes faster recovery from illness, prevents unnecessary hospital admissions, supports timely discharges and maximises independent living.

Intermediate care is targeted at people who face unnecessary prolonged hospital stays or avoidable admissions to hospital, residential or continuing care.

The intermediate care team will work with you to set achievable goals and plan your care to enable you to become as independent as possible. They will devise a Rehabilitation programme to suit your needs. Where you will be encouraged to: participate in activities which you would normally do at home such as making your bed and preparing your meals, dressing and undressing and attending to personal hygiene. You will also be expected to take part in group activities and one to one sessions when appropriate.
