Crisis Response Team – South Manchester
The South Manchester Crisis Response Team supports people who need urgent support at home, to prevent unnecessary hospital admissions.
The service accepts referrals from community health, social care and the ‘front door’ of the hospital (A&E).
The team also work with the North West Ambulance Service (NWAS), to accept referrals directly from paramedics and crews on site, to avoid a transfer to hospital.
The team provides rapid assessment and support planning for people with an urgent health or social care need that could lead to a same-day admission to hospital.
We support people to remain at home while the crisis situation is addressed.
Our multi-disciplinary team includes:
- Advanced Clinical Practitioners
- GP’s
- Nurses
- Occupational Therapists
- Physiotherapists
- Social Workers
- Pharmacists
- Health Care Support Workers
We provide urgent assessments and interventions for people who have a health or social care crisis. We provide clinical support for up to 72 hours.
Referrals are triaged and allocated to the relevant person in the team and arrangements made to assess the patient as soon as possible.
The referrals from NWAS will be triaged by an ACP.
We will carry out a comprehensive assessment of needs and provide treatment and support, to enable them to stay at home
We aim to transfer care from the crisis team within 72 hours – after the immediate needs are addressed. After that, we can refer to other appropriate services and support as needed.
What happens on discharge?
If the person needs ongoing support, the team will, with consent, arrange referrals to other community services that can support them at home. We will write to the person’s GP with a discharge summary, so that the GP is aware of the care received from the team.

The service is for consenting adults 18+ years, who are registered with a South Manchester GP. This is an integrated health & social care team, however, services provided by Manchester City Council (social care/reablement/equipment) are available to Manchester residents only.
Referral to the team is not condition-specific, but for people who need an urgent assessment of need to remain safely at home, and where a hospital admission is not required or can be avoided.
Referral exclusions:
- Acute mental health
- Direct pregnancy related issue
- Under 18
