Community Rehabilitation – North Manchester

The Community Rehabilitation service sees patients in their own homes (as well as nursing homes) and provides a comprehensive assessment of rehabilitation needs to enable patients to gain maximum independence with activities of daily living and mobility.

We aim to promote independence in activities of daily living, maximising mobility and function in a safe environment.

The Community Rehabilitation team work with adults registered with a North Manchester GP who requires rehabilitation in a variety of settings including home, residential or nursing homes.

We are a team comprising of Physiotherapists, Occupational Therapists, Pharmacist, GP Extensivist and Rehabilitation Assistants, with specialist knowledge and expertise in Community Rehabilitation.

We offer treatment for a variety of general conditions including falls and work with patients to achieve their goals.

The Occupational Therapy element of the service offers assessment of daily routines, depending on identified problems. They can offer rehabilitation using activities to enable people to function more fully around the home i.e. personal hygiene, preparing meals, mobility and providing equipment and minor adaptations. It also offers advice and support and education on using different techniques i.e. coping strategies such as fatigue management and energy conservation.

The service also provides falls strength and balance classes in the community.

You will be visited at home and assessed by a qualified and HCPC registered Physiotherapist or Occupational Therapist. Treatment may continue with this therapist or may be handed over to a rehabilitation assistant with regular reviews if deemed appropriate.  With consent, patient’s case can then be discussed with a specialist GP and pharmacist, at our weekly meeting if any outstanding medical issues need advising on and addressing.
