North Manchester Community Crisis Response provides a rapid response and assessment service in the community for people in urgent need of health and social care intervention at home.

The team work to respond quickly to keep people who are in a crisis at home with the right level of treatment and support from an expert multi-disciplinary team of professionals.

The team will take calls from the ambulance service attending emergency calls at a patient’s home, primary care (GPs) when a patient presents at a GP practice or a home visit, and even hospital A&E departments to respond to patients who would otherwise need to go to A&E and into acute hospital care. Community health and social care teams can also refer patients to the team.

The service aims to prevent avoidable hospital attendance and admission by responding rapidly to calls and keeping people safe in their own residences.

The community crisis response service is a successful model that has been in place across community services in Manchester for several years.

The service is designed to help anyone over the age of 18 and registered with a North Manchester GP who is experiencing a crisis at home. They must be referred to the service by one of the professional listed above.

Patients using the service would likely otherwise go to A&E, and the majority would likely be admitted to hospital as a result.

Patients must be over 18. Referrals are not limited to specific conditions, but some of the common conditions crisis teams support include:

  • Fall with no apparent serious injury
  • Exacerbation of existing condition
  • Reduced function / deconditioning / reduced mobility
  • Confusion/delirium
  • Palliative/end of life care crisis support (if not currently under the care of another service)
  • Provision of equipment to prevent hospital admission.

Community Crisis Response cannot treat people in an acute mental health or substance abuse crisis, or with issues relating directly to pregnancy.

Community Crisis Response team will take calls from NWAS, community health and primary care — responding quickly to support those who are in a crisis at home with the right level of treatment and care from an expert multi-disciplinary team.

Our multi-disciplinary team includes:

  • Advanced Clinical Practitioners
  • GP’s
  • Nurses
  • Occupational Therapists
  • Physiotherapists
  • Social Workers
  • Pharmacists
  • Health Care Support Workers

The team will respond within two hours of receiving the call, attend the patient’s home (or place of residence) and provide up to 48 hours of input to stabilise and support before handing over to mainstream community health, social care and other appropriate services.

Referrals are accepted from primary care, community health, social care, NWAS crews attending calls and hospital A&E to prevent avoidable admission. Patients cannot self-refer to this service.

  • Patients must be registered with a North Manchester GP

The service CANNOT support:

  • People under the age of 18
  • People with acute mental health or substance abuse crisis
