Community Rehabilitation Service – Citywide

The Community Rehabilitation Services cover both Manchester Local Care Organisation (MLCO) and Trafford Local Care Organisation (TLCO) footprint with services based in each locality, i.e. North, Central, South and Trafford. The Community Rehabilitation service sees patients in their own homes (as well as nursing homes) and provides a comprehensive assessment and treatment of rehabilitation needs to enable patients to gain maximum independence with activities of daily living and mobility.

Our Community Rehabilitation team includes:

  • Physiotherapists – supporting people who have difficulty with movement, strength and balance.
  • Occupational therapists – focusing on improving people’s ability to perform the tasks that they need to do in their everyday life.
  • Rehabilitation assistants – supporting patients to carry out their therapy.

We aim to promote independence in activities of daily living, maximising mobility and function in a safe environment.

Community rehabilitation service is a non-urgent therapy service for adults (18+) who are registered with a Manchester GP and are in need of therapy input in the community. Patients for whom therapy input in the community is appropriate and require treatment and management in the home environment or community. I.e. patients whose needs cannot be met in an out-patient setting.

You must be registered with a Manchester GP to access the service. If you are registered with a North Manchester GP, complete the North team referral form, a Central Manchester GP, complete the central team form and so on.

To promote people’s independence doing everyday tasks safely, we can provide:

  • Exercises to improve mobility, balance and strength
  • Strategies to support independence with daily living such as getting washed and dressed, preparing food and drinks, household management
  • Falls assessment, prevention and management advice
  • Provision of equipment such as mobility aids and kitchen/bathroom equipment to support rehabilitation
  • Strategies for managing fatigue and anxiety
  • Advice and education for patients and their carers/family to help manage their own health conditions
  • Signposting to local community activities, groups and exercise programmes.

We accept referrals from any health care professional or patient or patient’s carer. It is important all sections of the referral form are completed. Incomplete Referrals will be returned to the sender. Patients can ring single point of access number to self-refer. Referrals are triaged by a clinician. The referral will either be accepted, redirected to appropriate service or rejected. Accepted referrals will be triaged to a waiting list according to triaged priority. Rejected referrals will be sent to the referrer with a reason for the rejection.

The service is provided Monday to Friday, excluding bank holidays between hours of 8am – 4 pm.
