About Community Care Navigators

We are a non-clinical service. We help people navigate access to care and support in the community.

We work with you, connecting you to the right service that is available to you, at the right time, depending on your support needs.

We link in with other health and social care professionals to help you access the support you need. This includes:

  • GPs
  • Social workers
  • Home adaptations team
  • Community nursing.

We also link with lots of other community resources local to you, including local charities, voluntary groups and social enterprise organisations.

Care Navigators work with people to ensure they are connected with the right services available to them in your local neighbourhood. We work closely with NHS services, mental health services, GP’s, social services (council), housing associations, charities, voluntary groups, social enterprise organisations and other community resources to do this.

We listen to what’s important to you.

We can discuss your health and social care needs in a place of your choice such as your home, local community hub or GP surgery.

We can also talk over the phone.

We work with anyone who is 18 years old and above.

They must have a Manchester or Trafford GP or live in Manchester or Trafford.

Our service is available 5 days a week:

  • Monday to Friday, 9am – 5pm

Who provides this service?

This service is provided by Manchester Local Care Organisation (MLCO). MLCO is the organisation that provides NHS community healthcare and adult social care in the city. It is a partnership organisation between the NHS and Manchester City Council.
