Macmillan Services – Scheduled Care – Central Manchester

We are a team of highly expert health professionals who have undertaken specialised training to care for patients and relatives who have cancer and other life limiting illnesses.

We work closely with district nurses, GP’s and many other health professionals to provide expert care and emotional support.

We mainly visit people with a diagnosis of cancer, but also anyone with a life limiting illness registered with a GP and living in Central Manchester.

Admission:

Open referral system for patients with a life limiting illness, who require specialist palliative care and are registered with a Central Manchester GP. Patients are able to self-refer, and we also accept referrals from GPs, District Nurses and Secondary Care.

Service available 8:30am -5pm, Monday- Friday and 9am – 5pm Saturdays and Sundays

Staff:

The team consists of Medics, Clinical Nurse Specialists, Associate Clinical Nurse Specialists, a Physio, Occupational Therapist, Speech and Language Therapist, Dietician, Assistant Practitioners and Administrators/Medical Secretary.

Conditions:

Cancer and other advanced life threatening illnesses.

Referrals:

Referrals via GP, District Nurse, Self Referral or Hospital Team or ring to discuss with team.
