Crisis Response Team – Central Manchester

The Central Manchester Crisis Response Team support patients who need urgent support at home, but who do not need to be admitted to hospital. The team accept referrals from North West Ambulance Service (NWAS) and from community refers such as GPs, social services and community health teams.

The team provides urgent assessments and interventions for people who have a health or social care crisis. We support people to remain at home rather than go to hospital while the crisis situation is addressed. We aim to provide care for up to 72 hours.

People who:

  • are over 18 years of age
  • are registered with a Central Manchester GP
  • has given us their consent

The team is made up of specialists from health and social care including:

  • Advanced Clinical Practitioners
  • Nurses
  • Occupational Therapists
  • Physiotherapists
  • Social Workers
  • Pharmacists
  • Support Workers.

On attending a call/seeing a patient that meets agreed criteria, the ambulance service/GP/community service will contact us (with the patient’s consent). We then contact the patient by phone and arrange to visit them at home.

The team will carry out a comprehensive assessment of needs and:

  • provide treatment and support to enable them to stay at home
  • provide essential equipment.

We aim to discharge from the crisis service within 72 hours – after the patient’s immediate needs are addressed. After that, we can refer to appropriate services and support as needed.

What happens on discharge?

If the patient needs ongoing support we will, with their consent, arrange referrals to other community services that can support them at home. We will write to their GP with a discharge summary so they are aware of the care received from the team.

Referral to the service is not dependent on any specific condition or illness, it’s for people who need urgent support at home, but who do not need to be admitted to hospital.

We only accept phone referrals.

Opening hours

Monday to Sunday, 8:30am to 10:00pm.
The last referral to our service is taken at 8:30pm to give us time to respond.
