DAFNE is a structured diabetes education programme in intensive insulin therapy self-management for adults (aged 17 and over) with type 1 diabetes.
The course formats are : Online learning with virtual group sessions (Remote) & Face-to-face delivery
Online format
- Remote DAFNE – one day a week over five weeks
- Remote Pump DAFNE – one day a week over five weeks
If you attend DAFNE online, it will take five weeks to complete, there’s also an online refresher course available for all DAFNE graduates.
Face to face format
A face-to-face DAFNE course is run over five days – this can be either over one week (Monday to Friday) or spread over five weeks. DAFNE covers things like carb counting, insulin dosing and blood glucose management, but also, how you manage your type 1 diabetes around things like exercise, illness, and social activities.

BERTIE – Beta Cell Education Resources for Training in Insulin and Eating

BERTIE is a structured diabetes education course that teaches people with type 1 diabetes how to manage insulin doses compared to carbohydrate intake. It was created by the Bournemouth Diabetes and Endocrine Centre (BDEC) with the aim of teaching patients how to recognise and count carbohydrates, how to work out insulin doses, and how this affects blood glucose levels.
Anyone with type 1 diabetes who is over the age of 16 can attend the BERTIE course, while patients with type 2 diabetes who are treated with multiple daily injections (MDI) will also be considered.
When to take up offer – At any point after diagnosis of type 1 diabetes
What does the BERTIE course involve – The BERTIE course involves attending a 6-hour group programme once a week for 4 consecutive weeks. It is led by a diabetes specialist nurse (DSN) and a diabetes dietitian. As well as learning how to carb-count, and adjust insulin doses accordingly, participants will keep records of food intake, blood glucose levels and insulin administered between sessions.

How does the patient/person get onto the programme – Patients will consult their GP, DSN or consultant if they would like to be referred onto the BERTIE course. DigiBete is a collaboration between a clinical team and a parent led initiative by Leeds Teaching Hospitals NHS Trust.
The aim is to increase Education, Awareness and Training for Type 1 but is not a substitute for diabetes specialist HCPs.

- It is a free to access site for everyone; no registrations, no payment and the content has been developed/co-designed by parents, clinicians, and the type 2 community.
- In the app you can favourite films you find most useful and save them to your device, store insulin ratios/doses and pump settings, add appointments, add a school healthcare plan and more.
- Clinics can also send out useful updates and news directly to their app users.
- The app is not a substitute for care from clinics but helps complement the care they provide.

MyDESMOND
Type 2 Diabetes Management – Digital programme for people with Type 2 diabetes.
Designed to support self-management through digital means with modern lifestyles at the heart.
Primarily developed with mobile-use in mind, it has a responsive website and can be used on tablets and PCs.
There are currently 3 digital diabetes programmes available on the MyDESMOND platform:
- Type 2 diabetes – suitable for anyone with a type 2 diabetes diagnosis
- Let’s Prevent Diabetes – for individuals who find themselves at risk of developing type 2 diabetes
- Babysteps – a prevention programme specifically designed for women who had a previous diagnosis of gestational diabetes

NHS Type 2 Diabetes Path to Remission Programme

The programme – A low calorie treatment for people living with Type 2 diabetes who are above a healthy weight.
This expert-developed 12-month programme has 3 phases:
- To start with you’ll follow a low-calorie diet for 12 weeks (replacing all normal meals with your choice of soups, shakes and porridges totaling 8-900 calories per day).
- Then you’ll gradually restart ‘real food’ meals over 6 weeks.
- Finally, you’ll receive ongoing support for the last 8 months to help you maintain or build on your weight loss.
You’ll receive high-quality resources and one-to-one support from trained Coaches.
These will give you the knowledge, skills and support you need throughout the programme and afterwards.
You can choose whether to access your Coach support in-person at a local venue or digitally over an app.
Once you provide the safety monitoring information, you’ll be given all your total diet replacement products and a range of high-quality resources including Workbooks, a Lifestyle logbook, online Recipe book and 12 months’ premium access to the EXi physical activity – all free of charge.
Throughout the programme you’ll continue to be supported by your GP practice – for example if your medicines need to be changed
Eligibility
- Registered with a GP practice in Greater Manchester
- Aged 18-65 inclusive
- Diagnosed with Type 2 diabetes within the last 6 years
- Have a Body Mass Index (BMI) of at least 27 (or 25 if you are of South Asian or Black African or Black Caribbean ethnicity)
- Attended your monitoring and diabetes review, including retinal screening, if you’ve been offered it
- Have an HbA1c (blood glucose) level of:
43-87mmol/mol if on diabetes medications
48-87mmol/mol if NOT on diabetes medications

Healthier You – NHS Diabetes Prevention Programme (NDPP)

Healthier You: NHS Diabetes Prevention Programme is free to people at risk of Type 2 diabetes.
The programme is designed to be informal, interactive and supportive, and there are three ways to take part in the programme: Face-to-Face, Digital and Remote.
Face-to-Face
13 friendly and supportive group-based sessions over 9 months with a trained Health Coach.
How it works
A trained Health Coach will help you make a number of simple and sustainable changes to your lifestyle. You will be supported through a series of behaviour change ideas in a group. Over next 9 months you will attend 13 sessions. The first four sessions are fortnightly, then monthly from then on.
Session 1
You will meet your Health Coach and group. Fun and interactive support and advice sessions will help you start your journey
Session 2 – 4
With your Health Coach, you will set realistic goals and learn to build balanced meals, increase your activity levels, tackle negative thought patterns and setbacks. These will be small, incremental changes to a healthier you.
Session 5 – 13
You will learn to sustain your new lifestyle. You will return to your GP to have another blood test and look for an improvement in your risk of developing Type 2 Diabetes.
Digital
Core Phase (Weeks 1-12) Participants access structured information and activities
On average, each participant will receive 2 hours of dedicated Coach time per week, equating to 50 one-to-one and 75 weekly group messages. Sustain Phase (Weeks 13-40) Participants access content that embeds behavioural change. On average, each Participant will receive 30-60 minutes of dedicated weekly Coach time, equating to 15 one-to-one and 15 weekly group messages.
How it works
You will use the Second Nature app to take part in the programme on your phone. The app will support you through a series of behaviour change ideas with access to a support group and Health Coach. Over next 9 months you will work your way through the programme to build and maintain healthy habits in the long term.
Getting Started
An Adviser from Second Nature will call you. They will talk to you about the app, the support you will receive and answer any questions you have.
Weeks 2 to 13
You will meet your Health Coach and support group through the app. Together, you will set realistic goals and learn to build balanced meals, increase your activity levels, tackle negative thought patterns and setbacks. These will be small, incremental changes to a healthier you.
Weeks 14 to 40
You will learn to sustain your new lifestyle. You will track your progress using the app and your scales. When you complete the programme, you will have a final consultation to record your new BMI.
Remote
In online video sessions with a Health Coach if you need additional support with access.
How it works
They will help you make a number of simple and sustainable changes to your lifestyle. Over next 9 months you will attend 13 sessions. The first four sessions are fortnightly, then monthly from then on.
Visually Impaired
Your sessions will be in maximum groups of eight, with support provided by RNIB.
Sessions will be hosted on Zoom.
Hearing Impaired
Your sessions will be in maximum groups of eight, with support provided by Clarion.
Sessions will be hosted on Clarion’s Video Platform with BSL interpreters.
Bangladeshi/Pakistani
Your sessions will feature cultural adaptations, with support provided by Language Line interpreters. Sessions will be hosted on Zoom and available in Urdu, Punjabi, Bengali, Pashto and Gujarati.
Women with Previous Gestational Diabetes
Your sessions will be adapted to incorporate appropriate recipes and exercise. Sessions will be hosted on Zoom.
