Central Manchester services

Welcome to Central Manchester’s Integrated Neighbourhood Teams.

We've been working in our four integrated neighbourhood teams (INTs) since 2015. All INT’s will soon be co-located providing the right care at the right place.
 
Over the last few year we've been starting to work in an integrated way along with our Intermediate care colleagues. We're building on the learning from these two services. Our practice-based multi-disciplinary team (MDT) meetings (where staff from different disciplines come together) have been established as part of our approach.
 
We've started to develop processes and procedures for each INT and we're looking at how to involve administrative functions, primary care and mental health.
