The Active Case Management Service (ACM) supports patients with long-term conditions (LTCs) in the community in their own homes.
The ACM service is an integral part of the Integrated Primary Health Care Team and wider health care system. The aim of the service is to care for patients with complex needs, through intensive co-ordination, at a complex health and social care multi-disciplinary team level.
This is a non emergency service which specialises in helping people with long term conditions, it is part of the Integrated Neighbourhood Team. Patients referred to the service must be registered with a North Manchester GP.
Healthcare professionals who are experienced in the management of long term conditions. A 12 week period of holistic assessment which aims to empower patient to identify own goals and be proactive rather than reactive.
