IV-Adult Community Therapy Team (IV-ACT Team) – Central Manchester

The IV Adult Community Therapy Team (IV-ACT team) is a CCVG commissioned service looking at the benefits of treating patients at home or in a community setting with IV antibiotics and subcutaneous fluids.

We aim to support and treat patients at home or in the community.

Patient referrals from central Manchester GPs and surrounding hospitals as long as the patient is either resident in central Manchester or has a central Manchester GP.

The team will support hospital patients at home who need monitoring by taking blood tests and other samples to enable out-patient review to be quicker.

The team have also taken on intravenous therapies other than antibiotics, for example infliximab and furosemide and are looking to expand this new work.

The first point of contact is usually a telephone call to the service asking if the patient meets criteria for referral. Once this has been established the referrer will fill in a simple referral form, prescribe the medication and then the IV team will do the rest.

The IV ACT team are based at Gorton Parks Nursing Home but also have a base at MRI. They accept patient referrals from central Manchester GPs and surrounding hospitals as long as the patient is either resident in central Manchester or has a central Manchester GP. The team also facilitates discharges of patients who live out of area by liaising with other IV services within the region. Some out of area patients can be taken by the team but this is on a case by case basis.
